Change of Program Request Form
As a general rule, requests for a Change of Program are not processed after the 13th week of the semester (sixth week of the summer session) until the Graduate College receives student grades for that semester. A student who is on any type of probation or who is in their first semester of a conditional or Unclassified admission is not permitted to change his or her major.
NOTE: If you meet any of the following restrictions, you must re-apply for admission through the Office of Admission. A Change of Program request is not appropriate for your situation.
- You were admitted to the Graduate College, but you never enrolled. Now you would like to update your original admission to reflect a different semester.
- You were admitted to the Graduate College, but you never enrolled. Now you would like to update your original admission to reflect a different program and semester.
- You have not enrolled as a graduate student in the last year (3 semesters, including summer).
- You have completed your present graduate program and a major semester (fall or spring) has elapsed since your last enrollment.
