Addition or Change of Program Request Form
As a general rule, requests for
changes of program are not processed after the 13th week of the semester
(sixth week of the summer session) until the Graduate College receives
student grades for that semester. A student on any type of probation
or in their first semester of a conditional admission is not permitted to
change his or her major.
